As of August 1, 2014, ActiveCare Medical Services converted to a new model which is called the Kanata West Family Health Organization (FHO). This team is comprised of physicians from both the Kanata and Carlingwood locations. The foundation of a Family Health Organization is based upon enrollment of patients with their family doctors. Most patients have already done this by previously enrolling in the Kanata West Family Health Group (FHG) and those patients will be transitioned over with no need to reenroll but for those who have not, we strongly encourage you to do so. Enrollment in the FHO will have many benefits for patients including access to our afterhours urgent care clinic. Patients are able to book same day appointments to see one of our doctors weekday evenings and weekend days for urgent problems only. Enrollment also provides nurse-staffed, after-hours Telephone Health Advisory Service to provide advice to enrolled patients after clinic operating hours. We ask you to refrain from using walk-in clinics or see doctors outside our group unless by referral from one of our FHO physicians. Many walk-in clinics do not send your records to your family doctor after the visit and doctors in another walk-in clinic do not have access to the medical history of people they are seeing. In a FHO, all doctors have access to an EMR (electronic medical records) system and as a result, can ensure the proper course of medical treatment is followed. While some patients may not notice any difference after the transition, we believe that it will greatly improve accessibility to medical care.
